چکیده

OBJECTIVEdMixed-meal tolerance test (MMTT) area under the curve C-peptide (AUCCP) is the gold-standardmeasure of endogenous insulin secretion in type 1 diabetes but is intensive and invasive to perform. The 90-min MMTT-stimulated CP $0.2 nmol/L (90CP) is related to improved clinical outcomes, and CP $0.1 nmol/L is the equivalent fasting measure (FCP). We assessed whether 90CP or FCP are alternatives to a full MMTT.

Comment on: Besser et al. Lessons From the Mixed-Meal Tolerance Test: Use of 90-Minute and Fasting C-Peptide in Pediatric Diabetes. Diabetes Care 2013;36:195–201

I t was extremely gratifying to read an interesting article in Diabetes Care by Besser et al. (1) that suggested that 90-min C-peptide is a highly sensitive and specific measure of area under curve and peak C-peptide in children and adolescents with type 1 diabetes and offers a practical alternative to a full mixed-meal tolerance test. The authors also suggested that C-peptide measured at diagn...

Urine C-Peptide Creatinine Ratio Is a Noninvasive Alternative to the Mixed-Meal Tolerance Test in Children and Adults With Type 1 Diabetes

OBJECTIVE Stimulated serum C-peptide (sCP) during a mixed-meal tolerance test (MMTT) is the gold standard measure of endogenous insulin secretion, but practical issues limit its use. We assessed urine C-peptide creatinine ratio (UCPCR) as an alternative. RESEARCH DESIGN AND METHODS Seventy-two type 1 diabetic patients (age of diagnosis median 14 years [interquartile range 10-22]; diabetes dur...

Response to Comment on: Besser et al. Lessons From the Mixed-Meal Tolerance Test: Use of 90-Minute and Fasting C-Peptide in Pediatric Diabetes. Diabetes Care 2013;36:195–201

We thank Dr. Chaudhary and colleagues (1) for their interest in our article (2). The authors raise some statistical questions about the analysis of the effects of age at diagnosis and fasting C-peptide on the time to insulin deficiency. We are happy that our analysis and results are correct but would like to clarify the assumptionsmade and tohighlight aminor issue with the presentation of these...

Predictors of incretin concentrations in subjects with normal, impaired, and diabetic glucose tolerance.

OBJECTIVE Defects in glucagon-like peptide 1 (GLP-1) secretion have been reported in some patients with type 2 diabetes after meal ingestion. We addressed the following questions: 1) Is the quantitative impairment in GLP-1 levels different after mixed meal or isolated glucose ingestion? 2) Which endogenous factors are associated with the concentrations of GLP-1? In particular, do elevated fasti...

Single-dose acarbose decreased glucose-dependent insulinotropic peptide and glucagon levels in Chinese patients with newly diagnosed type 2 diabetes mellitus after a mixed meal

BACKGROUND Acarbose slows down the intestinal absorption of carbohydrates, but its effects on the secretion of incretins are still poorly known. This study aimed to examine the effects of single-dose acarbose on the secretion of incretins in patients with newly diagnosed type 2 diabetes mellitus (T2DM). METHODS In this pilot study, twenty-three patients diagnosed with T2DM were randomly assig...
